Minimally invasive treatment of postoperative abdominal abscesses
Stupin V.A., Mikhailusov S.V., Moiseyenkova E.V., Drozdov G.E. Moscow. Postoperative abdominal abscesses and pelvic abscesses continue to have a high mortality rate (20.7–58.2%). The expanding scope of interventions and increasing surgical activity necessitate the search for new methods of prevention and treatment of postoperative purulent complications. Since 1989, the Clinic of Hospital Surgery No. 1 of the Russian State Medical University has been actively using the method of percutaneous puncture debridement of postoperative abscesses using ultrasound guidance (570 patients). Purulent complications have been verified after emergency interventions for peritonitis, intestinal obstruction, acute appendicitis, and destructive cholecystitis. Ultrasound scanning, and, when necessary, X-ray and magnetic resonance imaging were used to diagnose abscesses. A comprehensive examination identified an "acoustic window" that allowed for the use of minimally invasive puncture sanitation while avoiding injury to large vessels. During the initial phase of mastering the technique, puncture sanitation was used as a primary procedure before surgery in every tenth patient.
