Modern standards of diagnosis and treatment of various forms of rhinosinusitis.

Rhinosinusitis is an inflammation of the mucous membrane of the nose and paranasal sinuses (PNS), almost always caused by stagnation of secretions and impaired aeration of the PNS. The trigger for the development of rhinosinusitis is usually an acute respiratory viral infection (ARVI). An episode of acute sinusitis usually resolves within 2-3 weeks. Sinusitis is one of the most common diseases, which is recognized in almost all scientific works of the last decade. These studies have shown that approximately 10% of sinusitis cases are odontogenic in nature [1]. However, in Russia, the problem of rhinosinusitis exists only among practicing physicians and researchers, and is not considered at the state level of healthcare management due to the lack of official statistics [2]. At the same time, in the United States, for example, issues of rhinosinusitis epidemiology have been studied, such as the relationship of the disease with the patient's gender, place of residence, and social status. According to statistics, in the United States, sinusitis is more common among women than men, and among people from higher social strata [3]. This is due to regular visits to medical institutions and a careful attitude towards one's health among citizens with high incomes. Sinusitis is a leading pathology in otolaryngology, both in outpatient and inpatient settings. In recent decades, the incidence of sinusitis has increased almost threefold, and the proportion of hospitalizations increases annually by 1.5-2% [8]. On average, approximately 5-15% of the adult population and 5% of children suffer from some form of sinusitis [9]. When analyzing international data, the approach to the problem of rhinosinusitis in the United States turned out to be interesting, especially due to the similarity of climatic conditions in several territories of this country with Russia. According to the US National Health Survey, rhinosinusitis is the most common chronic condition, affecting 14.7% of Americans. According to the US National Center for Disease Statistics, sinusitis has become the most common chronic condition in the country, surpassing arthritis and hypertension in incidence. Almost one in eight people in the US suffers or has suffered from sinusitis. Therefore, the treatment of sinusitis is currently one of the main problems of otolaryngology, requiring colossal financial expenditures [1]. In the US, the costs associated with the diagnosis and treatment of sinusitis amount to 6 billion dollars per year. In addition to the economic component of the problem, there is also its social aspect. Rhinosinusitis significantly reduces the quality of life of people. Patients with rhinosinusitis have significantly worse pain sensitivity and social activity than patients with coronary insufficiency and chronic obstructive pulmonary diseases. Similar results have been obtained in Russia [2]. Recently, there has been a tendency towards an increase in the incidence of recurrent and chronic sinusitis, which is facilitated by a number of factors: 1. unfavorable environmental conditions in most regions of Russia, including those associated with natural factors; 2. increasing urbanization; 3. rapid development of such industries as food (preservatives, genetically modified products), chemical industry; 4. social: - smoking, - population migration. Factors that have a detrimental effect on the condition of the respiratory tract, reduce the body's defenses, affect the capabilities of specific and non-specific protection of the mucous membranes, including the mucous membrane of the nasal cavity. The resulting deficiency of local immunity leads to hypersensitivity of the mucous membrane, which can become a trigger in the development of sinusitis [9]. Classification [4] The following clinical forms of sinusitis are distinguished: I. By duration of the disease — Acute sinusitis – OS (less than 4 weeks) — Subacute sinusitis – POS (4–12 weeks) — Recurrent acute sinusitis – RAS: - 4 episodes of acute sinusitis per year or more - each episode lasts 7 days or more - asymptomatic period lasts more than 2 months — Chronic sinusitis – CS (more than 12 weeks) II. By severity of the disease: — Mild: nasal congestion, mucous or mucopurulent discharge from the nose and/or into the oropharynx, body temperature up to 37.5°C, headache, weakness, hyposmia; on radiograph of the paranasal sinuses – the thickness of the mucous membrane is less than 6 mm. — Moderate: nasal congestion, purulent discharge from the nose and/or into the oropharynx, body temperature above 37.5°C, pain and tenderness upon palpation in the sinus projection, headache, hyposmia, pain may radiate to the teeth, ears, malaise; radiograph of the paranasal sinuses shows mucosal thickening of more than 6 mm, complete darkening or fluid level in 1 or 2 sinuses. —Severe: nasal congestion, often profuse purulent discharge from the nose and/or into the oropharynx (may be completely absent), body temperature above 38°C, severe tenderness upon palpation in the sinus projection, headache, anosmia, severe weakness; radiograph of the paranasal sinuses shows complete darkening or fluid level in more than 2 sinuses; In the general blood test - increased leukocytosis, a shift in the formula to the left, an accelerated ESR; orbital, intracranial complications or suspicion of them [5]. It should be noted that in each specific case, the severity is assessed based on the combination of the most pronounced symptoms. For example, if orbital or intracranial complications are suspected, the course is always assessed as severe, regardless of the severity of other symptoms. The diagnosis of recurrent acute rhinosinusitis in the USA and Europe is made when the patient has 3 episodes of acute sinusitis within a year or more, separated by asymptomatic intervals [6, 7]. Etiology and pathogenesis The main causative agents of acute sinusitis, according to Russian studies, are Streptococcus pneumoniae (48%) and Haemophilus Influenzae (12%), less often - Moraxella catarrhalis, Streptococcus pyogenes, Staphylococcus aureus, anaerobes. In POS and ROS, the spectrum and ratio of pathogens do not differ fundamentally from OS. The main etiologic role belongs to S. pneumoniae and H. Influenzae. In chronic sinusitis, anaerobes are of great importance, which are isolated in 48% of cases (Prevotella spp. - 31%, anaerobic streptococci - 22%, Fusobacterium spp. - 15%, etc.), aerobic pathogens are found in 52% of cases (streptococci - 21%, H. Influenzae - 16%, Pseudomonas aeruginosa - 15%, S. aureus, M. catarrhalis 10% each, respectively) [12]. It should be noted that recently there has been an increase in cases of rhinosinusitis, in which various genera of fungi are present [3]. Staph. aureus is not one of the most common pathogens causing sinusitis, as it is typically isolated primarily from the nasal vestibule and represents the flora present during specimen collection. However, this microorganism is more virulent and resistant to antibacterial therapy and is believed to cause the most severe cases of sinusitis and, significantly more frequently, inflammation in the sphenoid sinus (up to 29% of cases) [1]. RRS and HS may result from untreated OS associated with impaired mucociliary transport, pathology of intranasal structures, and iatrogenic causes. Recurrence and chronicity of sinusitis are facilitated by: immunodeficiency states (pregnancy, AIDS, post-transplantation state, chemotherapy, etc.), endocrine diseases (diabetes mellitus, hypothyroidism, etc.), allergic rhinitis, long-term use of vasoconstrictor drops, adenoids, a deviated nasal septum and other anatomical abnormalities, nasal polyps, tumors, dental infection, cystic fibrosis, Wegener's granulomatosis, sarcoidosis, Kartagener's syndrome, all those factors that contribute to a decrease in immunity, as well as the above-mentioned occupational hazards. Currently, the most popular theory of the rhinogenic cause of sinusitis development. As a rule, inflammation of the mucous membrane of the paranasal sinuses spreads from the nasal cavity, less often from odontogenic foci to the mucous membrane of the maxillary sinus. Inflammation of the paranasal sinuses as a result of trauma and a specific process is rare [8]. The onset of inflammation in the paranasal sinuses is in most cases preceded by an acute respiratory infection. Predisposing factors in the development of sinusitis are chronic diseases of the nose and nasopharynx. Normal functioning of the paranasal sinuses is mainly impaired due to: - blockage of the paranasal sinus orifices - concha bullosa (Fig. 1, 2), additional paranasal sinus cells (Galler cells, Onodi cells, etc.), hypertrophy of the uncinate process, curvature of the nasal septum, etc.; - impaired aerodynamics in the nasal cavity due to curvature of the nasal septum, hypertrophic, vasomotor rhinitis, resulting in inflammatory changes in the mucous membrane in the middle nasal passage, which again can lead to blockage of the paranasal sinus orifices; - impaired ciliary function; - production of excessive amount or change in viscosity of secretion, for example, in allergic rhinitis. Under conditions of normally functioning mucociliary transport, bacteria are unable to maintain prolonged contact with the epithelial cells of the nasal cavity and the paranasal sinuses. However, during a viral infection, when large or small areas of the ciliated epithelium affected by the virus are inactive, this prolongs the contact time between pathogenic bacteria and the cells and enables secondary bacterial infection. In addition to impaired mucociliary transport, structural abnormalities of the intranasal structures and the ethmoid labyrinth are important for the development of inflammation in the paranasal sinuses, causing aerodynamic disturbances in the nasal cavity and paranasal sinuses. This leads to obstruction of the natural openings of the paranasal sinuses and their aeration and clearance mechanisms. Congestion of secretions and decreased partial oxygen pressure in the paranasal sinuses create optimal conditions for the development of bacterial infection. In chronic inflammation of the mucous membrane, focal or diffuse metaplasia of pseudostratified columnar epithelium into multilayered epithelium occurs, devoid of cilia and, consequently, losing the ability to remove bacteria and viruses from its surface through active mucociliary transport. In viral infections of the upper respiratory tract, the edematous mucosa of the nasal cavity closes the opening of the sinus, disrupting its drainage. This leads to the absorption of oxygen from the sinus into the blood vessels of the mucous membrane. The resulting negative pressure promotes the release of transudate into the sinus, which serves as a breeding ground for bacteria penetrating through the opening of the sinus or by other means. Due to the inflammatory process, positive pressure develops in the closed volume of the sinus, accompanied by painful sensations [1]. Studies have shown that year-round allergic rhinitis contributes to the development of rhinosinusitis due to increased secretion production and blockage of the sinus ostia due to edema [11]. Diagnostics. Plain radiography is the most common method in the diagnosis of sinusitis. In most cases, plain radiography allows for the diagnosis of sinusitis. However, this method cannot be considered reliable in determining the nature of the lesion of the paranasal sinuses. Plain radiography does not provide clear data on the localization and nature of the pathological process in the sinuses, the state of the intranasal structures, in particular the ostiomeatal complex, or the spread of the pathological process into the cranial cavity or orbit [9]. Based on its data, it is possible to both overestimate and underestimate the severity of the ethmoid sinus lesion. In this regard, computed tomography is the most informative method [14]. The main advantages of computed tomography (CT) are that it provides a spatial representation of the relationship between the intranasal structures and the paranasal sinuses, allows one to judge the nature of anatomical abnormalities and their impact on the development of the pathological process, and evaluate the characteristics of tissues based on their X-ray density [9]. CT should be performed in the frontal and axial projections (Figs. 3, 4), as this allows for the best examination of the ostiomeatal complex, frontal sinus, and all other air cells. CT is best performed after adequate and sufficiently active drug therapy and in the absence of acute infection. Sagittal CT does not allow for the necessary assessment of anatomical features; it is mainly used to diagnose lesions of the sphenoid and frontal sinuses [14]. In the last decade, thanks to the introduction of modern optical systems equipped not only with high-resolution lenses but also with photographic and video equipment, endoscopy has become indispensable in the hands of otolaryngologists. Modern types of endoscopes, both rigid and flexible, allow for a high-quality examination of the nasal cavity, nasopharynx, anastomoses of the nasal passages, and even penetration into the sinus cavity. Endoscopy is indispensable in childhood, as it allows for a completely painless and trauma-free examination of the nasal cavity and nasopharynx for the child [9]. It should be noted that the data of anterior and posterior rhinoscopy, which are performed in the routine practice of otolaryngologists, and the data of endoscopic examination of the nasal cavity and NP differ significantly. During endoscopic examination, attention should be paid to a number of important signs: the presence of pus in the middle nasal meatus (Fig. 6); the presence of a pathologically curved middle nasal turbinate or concha bullosa; swelling and obstruction of the semilunar cleft, additional orifices and/or swelling or medial bulging of the uncinate process [14]. The intensive development of non-invasive research methods based on computer technologies has predetermined a completely new, qualitative leap in the diagnosis of NP diseases. Among such methods are acoustic rhinometry and anterior active rhinomanometry, which allow us to study the geometry of the nasal cavity and nasal resistance, and therefore reliably assess the functional capabilities of the nose. These methods are indispensable for objectifying the subjective sensations of both the patient and the physician, especially in assessing the results of surgical interventions on the nasal passages and intranasal structures, as well as conservative treatment of rhinosinusitis [9, 15]. Treatment The variety of functions performed by the mucous membrane of the nose and paranasal sinuses, the complexity of the pathophysiological processes occurring in the respiratory tract, requires that the physician beginning the treatment of diseases of the nose and paranasal sinuses adhere to gentle treatment principles aimed at effectively eliminating the pathological process, preserving or restoring the functions of the mucous membrane. To date, the leading principle of surgical interventions in rhinology has been radicalism. Excessive radicalism significantly impairs the protective function of the epithelium and the entire nasal cavity, and the olfactory function is impaired. The altered architecture of the nasal cavity severely disrupts aerodynamics, a crucial factor in air humidification and purification. Due to the disruption of transport, secretory, and calorific functions, dry, unpurified air begins to enter the lower respiratory tract, leading to dysfunction and diseases of the upper respiratory tract [8]. Furthermore, it is important to consider that predisposing factors in the development of sinusitis include chronic diseases of the nasal cavity and nasopharynx (such as a deviated septum (Fig. 7), hypertrophy of the nasal turbinates, etc.), which lead to a disruption of the drainage function of the natural sinus ostium. When the ostium closes, the removal of secretions from the sinus becomes impossible. To restore nasal breathing and drainage of the paranasal sinuses, treatment of chronic sinusitis must begin with eliminating pathological changes in the nasal cavity and nasopharynx that could contribute to the onset and maintenance of inflammation in the sinuses, i.e., with interventions on the nasal turbinates, nasal septum, and other areas. The most common intervention on the nasal turbinates is conchotomy. This procedure results in the loss of a large area and volume of mucous membrane. Wide nasal passages are formed, but aerodynamics are severely impaired, and the excretory, absorptive, and transport functions of the mucous membrane are impaired. This leads to "empty nose" syndrome. The inflammatory process persists, mucus and exudate dry on the surface of the mucous membrane, and the nasal cavity becomes crusted. Difficulty in nasal breathing reoccurs. Щадящим и высокоэффективным методом уменьшения объема носовых раковин является подслизистая вазотомия, при которой вмешательство производится подслизисто и слизистая оболочка раковины практически не страдает. При необходимости операция вазотомии дополняется смещением нижней раковины латерально. Нередко нижняя носовая раковина достаточно далеко отходит от латеральной стенки полости носа, соприкасаясь с перегородкой носа и вызывая затруднение носового дыхания. Пространство нижнего носового хода между нижней носовой раковиной и латеральной стенкой полости носа в дыхании не участвует, т.к. оно закрыто передним концом раковины. Смещение раковины латерально уменьшает это пространство, открывает общий носовой ход, улучшает аэродинамику и носовое дыхание. Операция производится одновременно с подслизистой вазотомией [8] Одной из наиболее частых эндоназальных операций является подслизистая резекция носовой перегородки. В клинической практике широко используется методика подслизистой резекции по Killian. Основным недостатком этой операции является необходимость удаления большой части костно-хрящевого остова перегородки и разрез по слизистой оболочке. Техника этой операции предусматривает вмешательство на хрящевой и костной частях перегородки, не затрагивая верхние отделы перегородки и самые нижние, как верхний латеральный хрящ или премаксилла. К недостаткам этой операции можно отнести следующие: 1. невозможно корригировать каудальный конец перегородки; 2. ограничен подход для реконструкции премаксиллы и передней носовой ости; 3. искривления области носового клапана не подлежат реконструкции; 4. подслизистая резекция неприменима у детей; 5. удаление значительной части хряща и кости создает условие для флотации перегородки; 6. после подслизистой резекции реоперация трудновыполнима; 7. может возникать перфорация перегородки. Большое распространение получила септопластика максилло-премаксиллярным подходом по Cottle. Максилло-премаксиллярный подход существенно расширяет возможности хирурга в операции на перегородке носа и ее реконструкции. Широкий хирургический доступ дает возможность продолжить диагностику состояния скелета перегородки носа и окружающих структур. Можно осуществить подход и исправить все отделы перегородки (рис.11) и прилежащих структур, совмещая операцию с реконструкцией пирамиды носа. Если необходимо, можно устранить разрывы и перфорации перегородки, как возникшие во время операции, так и ранее существующие. Максилло-премаксиллярный подход дает возможность подойти к любой структуре перегородки носа для ее реконструкции. Функциональная операция для восстановления носового дыхания этим подходом дает возможность одновременно реконструировать перегородку, пирамиду носа, верхние латеральные хрящи и нижние латеральные хрящи. С позиций щадящего отношения к структурам перегородки носа наиболее приемлемым является способ септопластики с реимплантацией аутохряща и аутокости между листками слизистой оболочки. Выбор непосредственно способа хирургической коррекции перегородки носа должен быть определен ситуацией и зависит от степени искривления перегородки и места искривления [22]. Самыми распространенными операциями на околоносовых пазухах до сих пор являются радикальные операции с удалением слизистой оболочки пазух. Однако уменьшение процента рецидива у больных после хирургического лечения радикальным способом наступает не всегда. Самый большой процент рецидивов наблюдается после радикальной операции на верхнечелюстной пазухе. Многие авторы отмечают, что соустье с нижним носовым ходом часто закрывается полностью, а если оно и остается, то воспаление не прекращается и врачам приходится применять различные способы для лечения воспаления. Одной из причин неудач радикальных операций на околоносовых пазухах является подход к лечению без учета патогенеза развития синусита. Распространение воспаления на слизистую оболочку околоносовых пазух в основном начинается из полости носа. В повседневной практике мы видим, что синуситу предшествует ринит и основная причина возникновения воспаления околоносовых пазух - риногенная. Хорошее дренирование пазухи может привести к нормализации даже грубо измененной слизистой оболочки при операциях на пазухах, поэтому тотальное удаление слизистой оболочки при операциях околоносовых пазух считается нерациональным.В профилактике и лечении синусита важно освободить соустье для хорошей вентиляции и удаления содержащегося в пазухах секрета, т.к. хорошая вентиляция не только улучшает мукоциллиарный транспорт, но и оказывает бактериостатическое действие. Полное удаление слизистой оболочки вызывает образование рубцов, неблагоприятно отражающееся на питании кости. В последнее десятилетие многие оториноларингологи считают методом выбора при лечении хронического риносинусита эндоскопические вмешательства на околоносовых пазухах. Основной принцип функциональной эндоскопической риносинусохирургии состоит в удалении только пораженных областей, чтобы устранить блок соустьев пазух и восстановить нормальное дренирование, вентиляцию и функционирование пазух. Вид вмешательства в основном определяется выраженностью поражения по данным КТ ОНП.[8]. Перед операцией необходимо провести предоперационную подготовку больных. Проводится пероральная антибиотикотерапия для купирования обострения воспалительного процесса. Около 90% возбудителей воспалительных процессов в околоносовых пазухах бывают чувствительны к амоксициллину и ингибитору бета-лактамазы – клавулановой кислоте. Проведение общей антибиотикотерапии можно сочетать с местным лечением антимикробными препаратами или антисептиками слизистых оболочек. При сопутствующей бронхиальной астме и аспириновой триаде операция должна являться частью комплексного лечения. Хирургическое вмешательство в этой группе больных всегда следует выполнять после тщательного обследования, уточнения активности процесса и функционального состояния бронхо-легочной системы [21]. Послеоперационное ведение больных заключается в тщательном туалете полости носа, удалении корок, налетов фибрина, особенно из среднего и верхнего носовых ходов, а также из вскрытых полостей решетчатого лабиринта. При возможности исследование полости носа и околоносовых пазух должно быть выполнено при помощи гибкого и жесткого эндоскопа на 7 и 20 сутки. При осмотре оценивают состояние репаративных процессов, диаметр соустьев верхнечелюстных пазух, наличие в последних экссудата. Выявляют области избыточного гранулирования, отека слизистой оболочки, а также контактирующие противолежащие раневые поверхности, где могут образоваться синехии. При наличии в верхнечелюстных пазухах слизистого, слизисто-гнойного или геморрагического содержимого показано их промывание теплым физиологическим раствором или серебряной водой каждые два-три дня до полного очищения оперированных синусов. Эту процедуру выполняют через средний носовой ход при помощи канюли. При наличии отделяемого в лобных или клиновидных пазухах их промывают специальными изогнутыми канюлями. Одномоментное промывание всех пазух с одной стороны можно выполнять при помощи синус-катетера ЯМИК. Регулярное удаление содержимого способствует скорейшей ликвидации послеоперационного отека и воспаления в оперированных пазухах. Больным следует рекомендовать самостоятельно промывать полость носа теплым физиологическим раствором 2 раза в день в течение 3-4 недель после операции. При сохраняющемся воспалительном процессе, большом количестве слизисто-гнойного или гнойного отделяемого в оперированных пазухах наряду с регулярными промываниями необходимо применение местной или системной антибактериальной терапии. Больные должны находиться под наблюдением до тех пор, пока полностью не исчезнет экссудат из всех оперированных пазух и не наступит эпителизация всех раневых поверхностей. Большую роль в уменьшении рецидивов хронического риносинусита после хирургического вмешательства играет местная кортикостероидная терапия [21]. Положительный результат функциональной эндоназальной операции лишь на 50% зависит от тщательности самого вмешательства, остальные 50% успеха – это следствие правильной подготовки больного к операции, грамотного ведения послеоперационного периода и тщательного выполнения пациентом всех рекомендаций [21].
