Possibilities of efferent treatment methods for polypous rhinosinusitis
K.B. Dobrynin, G.M. Portenko, Tver State Medical Academy. The aim of the study was to improve the effectiveness of anti-relapse treatment methods for polypous rhinosinusitis. The objectives of the study were to study and develop efferent treatment methods for anti-relapse effects in polypous rhinosinusitis. A modern approach to anti-relapse treatment of polypous rhinosinusitis (PR) aims to restore nasal breathing and prevent relapse. The main criterion for the effectiveness of anti-relapse treatment is the duration of the relapse-free period, the onset of which is assessed based on clinical and laboratory criteria. Another important feature is the removal of functional blockade, which develops with various medications. Repeated surgical interventions for PR lead to the development of varying degrees of functional blockade of various systems, which manifests itself as insensitivity or a distorted response to certain medications and therapeutic interventions. Considering PR as a manifestation of immune inflammation, it is necessary to use methods to correct defects in the immune system and correct structural changes to achieve an anti-relapse effect. The primary means for achieving this are efferent blood gravitation methods (therapeutic plasmapheresis, ultraviolet irradiation of autologous blood, blood ozonation). Relapses of PR most frequently occur in patients with concomitant upper respiratory tract pathology (bronchial asthma, chronic bronchitis with an asthmatic component, Fernand-Vidal syndrome), as well as in patients with polyvalent allergies. Sixty-eight patients with PR, aged 25 to 68 years, were examined. The disease duration ranged from 3 to 18 years, with frequent relapses of 1 to 3 times per year. All patients underwent a complete clinical examination: clinical examination of the patient with assessment of the state of vital functions, clinical analysis of the blood sample with determination of the main parameters: coagulation, determination of blood group and Rh factor, Wasserman reaction, antibodies to HIV, analysis of the blood group for Hbs-Ag and a-HCV, determination of blood protein (protein fractions), clinical urine analysis, study of the immune status, study of the blood cortisone level. Therapeutic plasmapheresis (LP) was performed in the postoperative period (nasal polypotomy, maxillary ethmoidectomy, endoscopic nasal polypotomy with infundibulotomy) on the CLG-3.5 and Sorvall devices. The course of treatment was 4 therapeutic surgeries. LP was performed in patients with PR with concomitant respiratory pathology. During a 3-year follow-up period, a relapse was noted in 1 patient with bronchial asthma (3.5%). We conducted treatment with ozonized saline (200.0) at a concentration of 2000 ?g / l by parenteral intravenous administration to 15 patients with polypous-purulent rhinosinusitis in the postoperative period. Ozonation was carried out using the Mesodon-BI device for 10-15 minutes. The treatment course is 5 procedures. With a follow-up period of 3 years, no relapse was noted. UFO of autoblood was performed in 25 patients with polypous-purulent rhinosinusitis in the postoperative period using the Izolda device, blood volume of 200 ml, irradiation area of ??35 sq. cm, perfusion rate of 10 ml / min. The treatment course is 5 procedures. With a follow-up period of 3 years, a relapse was observed in 1 patient (4%). The control group consisted of 25 people. Treatment effectiveness was assessed based on the clinical picture: restoration or improvement of nasal breathing, reduction of rhinorrhea, reduction of reactive edema of the nasal mucosa, improvement of general well-being, reduction of bronchial asthma attacks or a milder course of upper respiratory tract disease, reduction of eosinophils in the blood; according to indicators of stabilization of cellular and humoral immunity: increase in T- and B-lymphocytes, increase in the helper function of T-lymphocytes, increase in phagocytosis, decrease in circulating immune complexes, increase in the content of serum Ig A, M, G, decrease in the blood cortisone level. Thus, the clinical efficacy and good tolerability of the proposed efferent treatment methods for various forms of polypous rhinosinusitis allow us to recommend them for clinical practice.
