Results of the "Urological School of Thulium Technologies"

On March 1, 2018, the international scientific and practical seminar "Urological School of Thulium Technologies" was held. The event took place on the premises of the Scientific and Technical Association "IRE-Polus" and the State Budgetary Institution "City Clinical Hospital named after D.D. Pletnev (City Clinical Hospital No. 57). [ /upload/medialibrary/6c2/IMG_5892.JPG ] The School's program committee includes leading Russian and international urologists with extensive practical experience using thulium lasers. [ /upload/medialibrary/bd5/IMG_5828.JPG ] During the event, leading Russian specialists shared their experience of using a unique surgical device – the thulium laser “FiberLase U1” [ http://eleps.ru/katalog/lazernpaya-endourologiya/_apparat_lazernyy_khirurgicheskiy_povyshennoy_moshchnosti_fiberlase_u1_tu_9444_051_18003536_2015/ ] in various fields of application. For example, Sorokin N.I. - Head of the Oncology Urology Department at Clinical Hospital No. 2 of the First Moscow State Medical University named after I.M. Burnazyan of the Federal Medical and Biological Agency of the Russian Federation Martov A.G. described the possibilities of using a thulium laser in the endoscopic treatment of papillary tumors of the upper urinary tract, [ /upload/medialibrary/152/IMG_5934.JPG ] and Deputy Director for Research at the Research Institute of Uronephrology of the First Moscow State Medical University named after I.M. Sechenov, D.V. Enikeev, examined endoscopic enucleation of the prostate in his report. [ /upload/medialibrary/491/IMG_5999.JPG ] Invited guest from Germany, Professor Andreas Gross, chief urologist of the Asklepios Barmbek Clinic (Germany), spoke about the evolution of laser technologies in urology in his presentation. [ /upload/medialibrary/7e6/IMG_6047.JPG ] All participants of the School were able to clearly assess the capabilities of the thulium laser during demonstration operations. During the seminar, five online surgeries were performed using a domestic thulium laser: 1) A. Gross - Thulium enucleation of prostate hyperplasia (BPH); 2) D. V. Enikeev - Thulium enucleation of prostate hyperplasia with a Russian laser; 3) A. G. Martov - Thulium vaporization of papillary tumor of the upper urinary tract; 4) D. G. Tsarichenko - Thulium lithotripsy; 5) N. I. Sorokin - Laser resection of the bladder wall with a tumor in a single block (en-block). Visitors liked the fact that not only modern treatment methods were discussed, but also possible pitfalls in the form of complications and long-term results. It was noted that the "Urological School of Thulium Technologies" was especially useful for specialists with little or no experience working with laser equipment, since thanks to the interesting, detailed and understandable work of specialists performing operations, the fear of new equipment and a new operation disappears. For those with some experience working with laser technologies, the event was useful in that it helped to form a holistic picture of the correct use of lasers, learn about new trends and understand the best suited for each method of exposure. Mikhail Vasilyevich Stranadko, Best Clinic, Moscow, urologist: "What impressed me about the thulium laser's capabilities is that we can clearly see the layers. We can separate the mucosa from the muscular layer without any negative consequences for the tissue." Artur Agasievich Saakyan, urologist at the Zhukovsky City Hospital: "I am most impressed by the versatility of this device. It can be used for urolithiasis, bladder tumors, and prostate diseases, which allows the surgeon to feel calm in the operating room and do their job as well as possible, in particular, treat patients." Yakov Borisovich Mirkin, Director of Research at the URO-PRO Medical Center in Krasnodar, noted: "This is new, but again, it depends on what you compare it to. If we compare it to holmium, holmium devices are very expensive. Thulium devices, if we are talking about a domestic device, are quite affordable. "You can compare it to diode lasers, which are cheaper. But still, diode lasers are a thing of the past; they don't perform the same functions as a thulium laser." Ramis Rafisovich Fatikhov, President of AMUR and Leading Researcher in the Endourology Department at the Urology Research Institute, said, "Thulium demonstrates the advantages of this type of energy. But what pleases me most is that we are no longer required to transfer patients to a renoprivate state. Today, this is the treatment of tumors of the ureters, kidneys, renal pelvis, and calyces, and what Professor A.G. Martov demonstrated today clearly demonstrates the capabilities of the thulium laser. I am very pleased that our patients will receive much higher-quality surgery. And this is the near future; it is a reality." Evgeny Sergeevich Gubanov, Head of Urology at the Samara State Medical University Clinic, said, "I was impressed by the surgical technique. In other words, the technique is interesting and similar to transurethral enucleation performed using conventional electric resectoscopes." But of course it is faster, more effective, more bloodless and modern. This is a different level, a level of efficiency, a level of safety for the patient and a different level of satisfaction for the doctor with his work.” A total of 103 specialists from all over Russia, mostly practicing urologists, attended the event.
